Question: I have been getting fillers in my smile lines, mouth corners and hollow cheeks for years. I am approaching my 70th birthday and seem to need more and more filler, as the years go by, to treat the same areas. It had gotten so costly to get filler injections every 9 to 12 months, not to mention that it’s painful to keep having these treatments so frequently. Is there a better injectable filler option? I don’t mind paying more for a longer- lasting filler.

Answer: What you are describing is referred to as “filler fatigue”. Most patients who have temporary fillers every year don’t continue with regular treatments for as long as you have. Studies show that filler fatigue can often kick in after just 2 or 3 repeat treatments.

An effective way to avoid the phenomenon of filler fatigue is to use a long-lasting filler. The long lasting options on the market include fat implants, stem cell injections and Bellafill, the 5-Year filler.

The downside of fat injections is that a good result typically consists of only 50 percent of the injected fat being retained. This means that adequate correction with fat injections can require serial treatments. Additionally, fat treatments can result in lumps when certain facial areas such as the lower lid hollows or fine lines are treated. Fat transplants also more invasive since they involve surgery with greater risks.

Stem cell fillers have become more known. These cells, which are harvested from various parts of the body, are then mixed with protein-rich plasma, or PRP, prior to injection. This technique also involves minimal surgery but is limited if you don’t have an adequate donor site from which to harvest the stem cells. Another downside is that after about age 53, stem cells may decline in function, resulting in better results for a younger population.

Of these 3 treatment options, I have found that Bellafill, the 5-year filler, can give the most reliable results, particularly if you are in your mid-50?s or older. Bellafill’s active ingredient works by stimulating collagen, no matter what your age. Older patients often ask me if they have any collagen left to stimulate. The fact is that, without collagen, your face and body would probably completely collapse. When younger, new collagen is produced at a faster rate. With age, this renewal process slows down, resulting in a higher amount of older collagen that is associated with wrinkles, lines, facial grooves and facial hollows.

I have performed over 3,000 syringes of Bellafill on patients ranging from 20 to 95 years of age. Unlike stem cell and fat graft injections, Bellafill is a non-surgical procedure and the least invasive of the 3 long-term filler options mentioned.
