Direct Neck Lift Incisions

Question: I read your article last Sunday about the direct neck lift. It seems like a good alternative for the right person. Unlike a facelift, the incisions in this procedure are in the front of the neck. Is this a newer procedure and how visible is the scar once it is healed?
Answer: The direct neck lift procedure has been around for decades. It wasn’t as popular in the last 20 years because the cosmetic industry has been trending towards minimal-invasive procedures with no scars at all. However, the pendulum has swung more towards the center as plastic surgeons strive to achieve a balance between non-surgical and surgical procedures in order to give patients the most optimal results.
While it’s true that, in the past, a drawback of this procedure was a more visible scar, advances in scar healing technologies have resulted in a resurgence of certain cosmetic procedures.
The direct neck lift incision is closed in a Z-plasty fashion. This special technique involves making the initial incision with tiny triangular flaps that interdigitate with each other. The Z-plasty incision is then sutured together in several layers, resulting in a less noticeable scar once healed.
The direct necklift procedure is followed by Infini RF treatments which are limited to the incision itself. Two Infini treatments are typically recommended with one performed four weeks after surgery and a second one at seven weeks after surgery. Infini RF stimulates collagen and elastin production to enable surgical incisions to heal with less visible scars.
Protein-rich plasma(PRP) is also applied to the tissues during surgery to further expedite the healing process and reduce scar formation. A small tube of blood is drawn from the patient’s arm and placed in the centrifuge to separate out the red and white blood cells, just leaving the protein-rich plasma. This PRP consists of numerous healing factors and other agents that can serve as messengers to further enhance collagen production. These agents are topically applied to the neck before the incisions are closed.
This contemporary approach of combining the Z-plasty technique with dual postoperative Infini RF treatments and intra-operative topical PRP application can greatly enhance results and reduce scar visibility, making the the direct neck lift a much more desirable procedure. Compared to a facelift or traditional necklift, additional benefits of the direct necklift include less invasive surgery, the use of local anesthesia in most cases and faster healing with less downtime.
