Errors in Filling Lower Eyelid Hollows

Question: My under eyes hollows were filled and now my eyes look puffy. The doctor was not a plastic surgeon but said the filler would dissolve with time. How can I prevent this in the future?
Answer: Correction of under eye hollows can be tricky. Knowledge of surgical anatomy, lower eyelid/cheek anatomy and anatomy of aging is key. An expert injector will also know which fillers are effective in treating lower lid hollows.
A common error I see in patients visiting my office for the first time is an overly filled lower lid hollow. This unnatural look is often due to poor injection technique, incorrect filler choice or a doctor’s failure to assess each patient’s unique anatomy of the lower eyelid/ upper cheek.
Determining if lower lid hollows are due to volume loss along the eye socket (orbit) itself or loss in the upper cheek area is crucial to the treatment approach. When volume loss occurs in both the upper cheek and orbit ridge, placing filler at the orbit rim alone can give a puffy, overfilled look unless the upper cheek is also treated. The areas of volume loss also impact choice of filler. Bellafill can safely and effectively restore volume loss in both the orbit ridge and cheek. Temporary fillers like Restylane and Juvederm do not last as long, but can also work as long as your surgeon makes you aware of their limitations in treating lower eyelid hollowing.
