Non-Surgical Treatment Of Sagging Neck

Question: I’m in my 50’s and have some sagging in my neck with loose skin and those vertical bands are just beginning to look visible in the front of my neck. I’m not ready for a necklift or any kind of surgery yet. Is there any non-surgical option for me at this stage?

Answer: While I cannot make a determination about your specific case without evaluating you in consultation, generally speaking, viable non-surgical options are available nowadays.

In patients with loose skin, excess fat and/or vertical neck bands, known as platysmal bands, I offer several approaches.

If you have mild to moderate neck skin sagging, a skin tightening “laser” device can be very effective. While there are many tightening devices on the market including Thermage, Ultherapy, Vivace, Venus, Thermo, Profound and Fractora, I have found Infini RF to be most effective due its advanced technology. Not only can Infini RF be used as a substitute for a facelift in cases of mild to moderate sagging, but it can also help maintain the results of a facelift or postpone the need for one.

When it comes to platysmal banding or vertical neck bands, significant ones typically require surgery. However, when the bands are mild or moderate, Botox can be very effective. Even if you have had surgery for the platysmal bands, these bands can

often reform due to neck muscle-related movements. Botox can be used to flatten these bands, resulting in a tighter neckline. Botox can also be effective in preventing these bands from getting worse.

In patients with excess neck fat, liposuction has been the traditional approach. However, not only is liposuction invasive surgery, but it can also leave loose overlying skin. Kybella is a permanent non-surgical, minimal-invasive solution to submental neck fat removal. Because some overlying skin contracture also occurs with Kybella treatment, I find that the overlying skin can also tighten in patients who are good candidates for Kybella.

Non-surgical neck rejuvenation may require one or a combination of all of three of these minimally-invasive treatments, depending on the unique aging needs of each patient.
